The CoR is structured into six specialised commissions, which are responsible for drafting opinions and analysing EU legislative proposals:
- CIVEX - Citizenship, Governance, Institutional and External Affairs
- COTER - Territorial Cohesion Policy and EU Budget
- ECON - Economic and Monetary Policy
- ENVE - Environment, Climate and Energy
- NAT - Natural Resources, Agriculture and Health
- SEDEC - Social Policy, Education, Employment and Research

The CoR plenary meets five or six times a year in Brussels to discuss and adopt opinions, adopt resolutions and express the regions' position on key issues on the European agenda.
Plenary sessions bring together the 329 members of the CoR and serve as a forum for debate on the future of the EU from a regional and local perspective.

In addition to thematic commissions, members of the CoR participate in intergroups and work networks, where they exchange experiences and discuss cross-cutting issues with other EU institutions.
Some of the key intergroups include:

- High-Level Group on Local Democracy and Decentralization
- Health Intergroup
- Covenant of Mayors for Climate and Energy
- Intergroup on Rural Development and Agriculture
- ARLEM Network
These forums allow regions to strengthen their presence in the EU and collaborate on strategic initiatives with other local and regional entities.
